Intravascular Lithotripsy with Diamond Drill in Iran

Breaks calcified vessels with shockwaves and a diamond drill

Intravascular lithotripsy (IVL) is an advanced and minimally invasive technique for opening narrowed blood vessels—especially in cases where hard calcium deposits have accumulated in the arterial walls. This method uses shockwaves to break these deposits, allowing blood flow through the vessels to become unobstructed. It is primarily used in treating:

  • Coronary artery narrowing due to calcium buildup, particularly when traditional methods are difficult
  • Peripheral artery blockages such as those in the limbs that involve calcification

Introduction

How does Diamond Drill Intravascular Lithotripsy work?

In IVL, a special catheter equipped with crystals that generate acoustic shockwaves is inserted through the artery into the narrowed vessel. The catheter delivers controlled shockwaves to the calcium deposits, breaking them up without damaging the healthy tissue of the arterial wall. As a result, the vessel is reopened, enabling stenting or restoring blood flow. The benefits of IVL include minimal invasiveness, precise targeting, improved blood circulation, and effectiveness for specific patient populations, among others.
Diamond Drill or Orbital Atherectomy – one of the most advanced treatments for severe arterial narrowing especially in patients with heavy calcification. This method uses a diamond-coated drill to grind down calcified plaques inside the vessel, paving the way for stenting or re-opening the artery.

Indications for Diamond Drill IVL

This method is used primarily for patients who:

  • Have severe narrowing of coronary arteries or peripheral arteries (e.g. in the legs)
  • Have hard calcium plaques in the vessel wall not responsive to balloon angioplasty or standard stenting
  • Have histories of diabetes, kidney failure, or advanced vascular disease
  • Need re-opening of an occluded stent.

Contraindications

Use of the diamond drill IVL is contraindicated or very high-risk in:

  • Presence of an active thrombus in the target vessel

  • Aneurysm at the site to be treated

  • Complete occlusion where the guidewire cannot pass

  • Unstable cardiac condition (e.g. simultaneous acute myocardial infarction)

  • Patients with severe allergy to system components or contrast agents

Treatment procedure with IVL

  1. Initial consultation and medical evaluation (online or in person)

  2. Imaging and angiography to assess severity of arterial narrowing

  3. Performing IVL by a specialist team in the catheterization lab (Cath Lab)

  4. Application of acoustic shockwaves to break up calcified plaques

  5. If needed, stenting after reopening the vessel

  6. Usually discharge within 24 to 48 hours
